The $0 Preventive Care Medicare Covers (That People Skip)

Medicare covers a long list of screenings, vaccines, and an annual wellness visit at no cost to you — no deductible, no copay. Yet many people never use them. Here's the preventive care you've already paid for, and how to actually claim it.

Care You've Already Paid For

Medicare Part B covers a substantial menu of preventive services at no cost to you — meaning no deductible and no coinsurance — as long as you use a provider who accepts Medicare and the service fits the covered frequency. This is care designed to catch problems early, when they're cheaper and easier to treat, and it's fully funded. Skipping it is leaving value on the table.

The catch is small but real: a preventive screening stays free only as long as it stays preventive. If a screening colonoscopy finds and removes a polyp, or a wellness visit turns into treating a specific complaint, the billing can shift from 'preventive' to 'diagnostic,' and normal cost-sharing may apply. That's not a reason to skip anything — it's a reason to know the distinction so a bill doesn't surprise you.

What's on the $0 List

The covered preventive services are broad. Among the ones people most often overlook:

  • The annual 'Wellness Visit' — a yearly planning visit (distinct from a full physical) to update your prevention plan
  • The one-time 'Welcome to Medicare' visit in your first 12 months of Part B
  • Cancer screenings: mammograms, cervical/colorectal screenings, and lung cancer screening for those who qualify
  • Cardiovascular screenings and diabetes screenings for at-risk patients
  • Vaccines: flu, pneumococcal, COVID-19, and — since 2023 — all Part D-covered vaccines including shingles, at $0
  • Bone density tests, depression screening, and counseling for tobacco, alcohol, and obesity

The Annual Wellness Visit Is Not a Physical

This trips up nearly everyone. Medicare's free Annual Wellness Visit is a prevention-planning appointment: your provider reviews your health risks, medications, and screenings, and builds or updates a personalized prevention plan. It is not a head-to-toe physical exam, and it doesn't include treating specific problems. If you bring a new symptom to it, that portion may be billed separately.

That's not a flaw — it's a genuinely useful yearly checkpoint, and it's free. Just go in knowing what it is: an annual tune-up of your prevention plan, not a full physical. If you want a comprehensive physical too, that's a separate visit with separate rules.

Use What You've Got

The practical takeaway is simple: book your Annual Wellness Visit every year, stay current on the screenings that apply to your age and risk factors, and get the vaccines — they're all free, and they're the cheapest health care you'll ever access. Preventive care is one of the few places in Medicare where the system is actively trying to give you something.
