How Much Is Medicare Per Month in 2026?

A realistic monthly cost for Medicare adds up several pieces. Here's what Part B, a drug plan, and a supplement or Advantage plan typically run, with examples.

The short answer

Medicare isn't a single monthly bill — your cost depends on which pieces you choose. Nearly everyone pays the standard Part B premium. On top of that, most people add either a Medicare Advantage plan (often low or zero premium) or Original Medicare with a Medigap supplement and a Part D drug plan (higher premium, lower costs when you use care). Higher earners also pay IRMAA surcharges.

So a realistic monthly number ranges widely based on the path you pick and your income.

What makes up your monthly cost

Start with the Part B premium, which most people pay. If you go the Original Medicare route, add a Medigap premium (which varies by plan letter, age, and location) and a Part D drug plan premium. If you go the Medicare Advantage route, you often pay little or no additional premium, but you'll pay copays as you use care. Our guide to the freedom-versus-cost tradeoff explains the two paths.

Two people the same age can pay very different monthly amounts depending on which structure they choose and their health usage.

Estimating your own number

The honest way to know your cost is to price your actual choices: the Part B premium, plus the specific supplement or Advantage plan and drug plan that fit your doctors and prescriptions. Our cost estimator helps you build a realistic monthly figure, and our breakdown of what Part A costs covers the rare Part A premium.

The takeaway: budget for the Part B premium as a baseline, then add the path you choose. Comparing plans on total monthly cost — not just premium — gives you the real picture.
