Mental Health Coverage Has Come a Long Way

Medicare's mental health coverage used to be an afterthought — limited providers, higher cost-sharing than physical health, and real gaps. That's changed substantially in recent years, with expansions that broadened the types of providers you can see and the services covered. If you assumed Medicare barely covered mental health, it's worth taking a fresh look, because the picture in 2026 is much stronger.

Mental health matters at every age, and for older adults facing loss, isolation, chronic illness, or major life transitions, access to counseling and psychiatric care is genuinely important. Here's what Medicare covers now.

What's Covered

Medicare covers a broad range of mental health services across its parts:

  • Outpatient therapy and counseling with covered providers, under Part B (you pay 20% coinsurance after the deductible)
  • Psychiatric evaluation and medication management

The 2026 Expansions

The recent expansions are what make today's coverage notably better. Medicare now covers services from a wider range of mental health professionals — including licensed marriage and family therapists and licensed professional counselors, provider types that weren't covered before. This dramatically increases the pool of therapists you can see and bill to Medicare, which matters enormously in rural Wyoming and Utah where mental health providers can be scarce.

Getting Mental Health Care Under Your Plan

To access mental health care: find a provider who accepts Medicare (now a larger pool thanks to the expansions), understand that outpatient care carries the 20% coinsurance under Original Medicare (covered by a Medigap plan), and take advantage of the free annual depression screening. On a Medicare Advantage plan, mental health is covered with the plan's copays and network — check that the providers you want are in-network.
